OKVision is a replacement contacts lens centre: we can
only supply you with lenses prescribed by your contacts lens
practitioner. OKVision must, by law, have verification of your
contact lens prescription before supplying your lenses.

Consequently we must ask you to supply us with the details of your
latest contact lens prescription. Please note your prescription is
valid for 24 months from the date of issue.

For your convenience, OKVision can verify your prescription
details with your Optician. See below for details.

Once OKVision has verified your prescription, your details
will be held on file until it expires (valid for 12 months).
This makes re-ordering lenses fast and efficient.

A contact lens prescription is more specific than a
regular spectacle prescription: in addition to the
particular lens and power you have been prescribed,
it also includes the lens fitting.


How to send your prescription details to OKVision:


1. Customers can post their current prescription to:

The original will be returned with your lenses.


2. Customers can fax their current prescription using
UK fax number 020 8632 1560 - if your order was placed
online using our secure shopping cart, please quote on
your fax your unique OKVision Order Number.


3. Alternatively, Customers can scan and email their current prescription.
Scanned images should be supplied in .gif or .jpeg format to info@okvision.co.uk


4. Submit your Optician's details online using the form below.
OKVision will then contact your Optician to verify your latest prescription.
